Thank you for referring to Chase Rehab for Occupational Therapy Services. We are a mobile service based in the Sutherland Shire. 

To help us begin working with the participant we have listed the below questions that will help us  identify the participants disability related needs along with their NDIS goals. To complete the referral form you will require: 

• Information on the participant who is requiring the service (if not you)
• NDIS plan, including plan dates, goals and funding options
